Minutes 2 Dec 2015

Minutes of Last Meeting:

Minutes of last meeting were read and it was agreed that no amendments were needed. There were no matters arising from the previous minutes.

Improvement of Patient Self-care:

It was agreed the group would like to look at the information available to patients to support selfcare. However as this is about to be taken on city wide it was agreed we would wait to see whether resources were made available to Practice’ s from the SCCG in this regard.

Age Concern/My NHS regarding the Vanguard scheme:

Mike and Dorothy had a number of questions arising from the meetings they had been involved with as part of patient engagement. Dr Gray explained the role of the new Community Integrated Teams and the Recovery At Home teams .She explained the different services which they would be providing, to help improve the care of patients with complex needs in the community and improve  the transition between hospital and primary care for patients who are being admitted and discharged from hospital.

Any Other Business:

Dr Gray brought a summary of complaints which the Practice had received over the following 12 months for discussion. There were four written complaints. Two of these had been settled locally .The others were in regards to other agencies .The Practice had passed these on to the relevant authorities.  It was agreed that no unifying themes had arisen from these complaints.

The friends and family test results for October were reviewed. Response rates were very low this month but still mostly positive. One included comments about a prolonged wait to be seen in the waiting room with a sick baby. Unfortunately as this was anonymous Dr Gray was unable to investigate this fully to ascertain whether there were any clinical reasons why the child should have been seen as a priority. All present accepted that sometimes when the Practice is busy and people wish to be seen urgently on the same day that this might entail a period of waiting to be fitted in.

Next meeting provisionally 8 March 6pm
